
Dear Therapist Colleague, Has this happened to you? Something unexpected comes up that requires you be away from your private practice office. You may feel, at first, a glimmer of relief that you have the flexibility to take care of the matters at hand. But immediately another one comes up - "Being away from the office means I don't get paid." My friend Eden was recently in an accident. Luckily she survived but she needed surgery on her face due to a number of broken bones. At first we discussed how lucky she is - not only to have survived the accident - but because as a self-employed therapist in private practice, she can set her own schedule around the surgery and her followup doctor's appointments. Eden is doing very well now.
However, she was unable to see her clients for almost two weeks. So that meant no income for a good part of the month. Plus there were numerous followup doctor's appointments. While Eden had savings to cover it, who wants to lose income at a time when they have unexpected medical expenses?
I get emails weekly from therapists who are thrust into the care-giving role for their families. With Helen it was taking care of her parents. Her mother had cancer and her father had dementia. She spent nearly a year seeing less than half her normal caseload - leaving for the office only when someone else could stay with her parents for a while.
Have you ever thought about that? We are in a profession where our job is to help people have better lives and relationships. We care for them until, hopefully, they learn to care more for themselves. But when we need time for self-care or to care for those close to us, what happens? We lose income.
That is our business model. We only get paid when we see a client.
When I think of it - this means our practices aren't really a business but a J-O-B. (Since that is the definition of a job - trading your time for an hourly fee.) And since we don't get paid sick time, vacation time, or have a retirement plan, I am not sure how great a job it really is.
This "trading one session for a fee" business model really becomes an issue in a downturn economy. For many clinicians, it has been a time of worry because the phone doesn't ring as often with new clients. Existing clients decide to spread their sessions further apart citing the reason "I simply can't afford it right now."
Isn't it time we address this model? Isn't it time to create a business that earns you money beyond the time you spend in one to one sessions?
But there is more. There are some additional reasons that now is the time to be thinking about creating an online business. There are some very specific sociocultural changes that are leading to new trends.
So what do we do? We need to get off that track (the one person for one hour for one fee track) and think more broadly about what we can offer the world and how to communicate it to as many people as possible. We need to share our wisdom and knowledge in ways that serve the community while leveraging our time and energy. We need to devise a model that supports us when the economy is booming and doesn't desert us when the economy is tanking. That business model is the Multiple Streams of Therapy Income Model. How does this Multiple Streams of Therapy Income model work? Simply put, this model helps clinicians augment their in-person practice with "information products"--books, CDs, audiotapes, e-books, and e-courses--created once and then sold repeatedly. The products may be physical, but the power of the model comes from using the Internet to sell and deliver them. Using this model meets consumer demand for instant gratification; lets therapists help people who can't afford therapy; extends therapists' reach beyond their local communities; and offers dependable, additional income. When implementing this model, some therapists choose to reduce the number of one-to-one hours and limit their in-person time to high-end workshops or retreats. Others choose to stop all clinical work and spend more time creating and selling products, or providing speaking and consulting services. The beauty of this model is that it can be adapted to each provider’s desires and personality. Let's Look at Your Marketing Funnel In a multiple streams of therapy income business, you create products and services that sell for different prices. For example, to get started, you offer a free product to help people get to know you and believe that you can help them. Then you create different products (or offer services) at various prices. This way you can appeal to buyers who are on a budget and to those that want to really invest more money in your higher priced one-on-one services or "bigger ticket" items. You create your content (for each product) once and sell it again and again via your website. By creating and offering products and services at different prices, you can appeal to many potential buyers. This is done by designing the products and services in your "product mix" - called the Multiple Streams of Therapy Income Funnel. Here are some examples of what you could put in your funnel. Of course you don't have to do all of these. But just imagine if you could create one or two products in each price tier in your funnel. You might create a free Top Ten List, a $20 CD, a $147 dollar 6-part teletraining program, $197 workshop, and a $500 weekend retreat, etc. Get the idea? The possibilities are endless and you get to use your gifts. If you prefer talking, you can create audios. Like to write? Create reports and books. ![]() It all starts with a mindset shift. Running a practice is different than running a multiple streams business. Creating an online business isn't rocket science but there is some market research necessary before you begin.

The pace will be slower - in terms of content delivery - because the participants will have homework between sessions.
